22 August 2016 - As medical science delivers once-unthinkable treatments available, patients in the US are increasingly facing a harsh reality: insurance companies are forcing them to try older, less expensive medicines for months before they are prepared to cover new, more expensive ones.

Insurers have long relied on a cautious approach to control costs and spare patients from expensive medications they might not need.

In more than a dozen interviews with doctors and patients, a picture has emerged of insurance companies growing more aggressive as they respond to financial pressures.
